Set Up T4 Summary Form window

The Set Up T4 Summary Form window enables you to enter information, including a company contact person and establishment number, on T4 Summary.

Field Descriptions

Name & Address

This field displays the name and address of your business. If you want to edit these details, click the zoom arrow next to the field.

Employer ID# or BN

This field displays your employer identification number or your business number, as it’s entered in the Business Number field of the Company Information window.

Proprietor's or Owner's SIN (Box 74)

Enter the Social Insurance number of the owner or proprietor of the business.

Proprietor's or Owner's SIN (Box 75)

Enter the Social Insurance number of the owner or proprietor of the business.

Contact Person (Box 76)

Enter the name, telephone number, fax number, and email address of the contact person you want to appear on your T4 Summary form.

Contact Person (Box 78)

Enter the name, telephone number, fax number, and email address of the contact person you want to appear on your T4 Summary form.

T4 Summary Info

Click to open the T4 Summary Information window, where you can view and edit the information that appears on your T4 Summary form.
